GUIDELINES ON HOW TO COMPILE THE REVISED APPLICATION FORM FOR THE EXPORT OF DUAL-USE GOODS AND MILITARY ITEMS
The application form for the export of dual-use goods and of military items has been revised in view of the imminent introduction of the new Import-Export System/Portal.
Below are guidelines of how to fill in the revised application form. Immediate compliance is required for newly submitted forms.
An asterisk ‘*’ indicates a compulsory field.
A separate form is to be submitted for each item.
Handwritten applications must be clearly legible.
BOX 1 EXPORTER
If the exporter is a company:
- VAT number* (required only if the company is registered in Malta)
- MFSA’s company registration number* (required only if the company is registered in Malta)
- Company name*
- Company address*
Contact person representing the company:
- Full name and surname*
- ID card number or Passport number*
- Telephone and/or Mobile number*
- E-mail address*
(If the exporter is a person acting in his own capacity, fill in the same details as indicated above for the ‘contact person representing the company’).
BOX 2 CONSIGNEE
If the consignee is a company:
- VAT number* (required only if the company is registered in Malta)
- MFSA’s company registration number* (required only if the company is registered in Malta)
- Company name*
- Company address*
Contact person representing the company:
- Full name and surname*
- ID card number or Passport number*
- Telephone and/or Mobile number*
- E-mail address*
(If the Consignee is a person acting in his own capacity, fill in the same details as indicated above for the ‘contact person representing the company’).
BOX 3 AGENT
If the agent is a company:
- VAT number* (required only if the company is registered in Malta)
- MFSA’s Company registration number* (required only if the company is registered in Malta)
- Company name*
- Company address*
Contact person representing the company:
- Full name and surname*
- ID card number or Passport number*
- Telephone and/or Mobile number*
- E-mail address*
(If no agent has been engaged, this section can be left empty).
BOX 4 END-USER
If the end-user is a company:
- VAT number* (required only if the company is registered in Malta)
- MFSA’s Company registration number* (required only if the company is registered in Malta)
- Company name*
- Company address*
Contact person representing the company:
- Full name and surname*
- ID card number or Passport number*
- Telephone and/or Mobile number*
- E-mail address*
(If the End User is a person acting in his own capacity, fill in the same details as indicated above for the ‘contact person representing the company’).
BOX 5 COUNTRY OF ORIGIN
The country where the goods were manufactured.*
BOX 6 COUNTRY OF CONSIGNMENT
The country to which the goods are being transferred and where they will be received by the consignee.*
BOX 7 COUNTRY OF FINAL DESTINATION
The country where the products will be used.*
BOX 8 MEMBER STATE OF CURRENT OR FUTURE LOCATION OF THE ITEM
The Member State of the European Union where the item is located at the time of the filing of the application. If this is not applicable, the first Member State that the item will go to during its transfer journey. *
(In the case of exports, re-exports and intra-community transfers, this will be Malta).
BOX 9 MEMBER STATE OF INTENDED ENTRY INTO THE CUSTOMS EXPORT
The Member State of the European Union where the item entered/will enter the export procedure. *
(In the case of exports, re-exports and intra-community transfers, this will be Malta).
BOX 10 DESCRIPTION OF GOODS
A detailed description of the goods, including:
- The full technical name of the item/s *
- Where applicable the Serial numbers / Chassis numbers / Model numbers / Engine numbers or any other means of unique identification of the goods under application.*
BOX 11 COMBINED NOMENCLATURE CODE
The Combined Nomenclature (CN) of the goods under application. This must be an 8 digit number. *
BOX 12 CONTROL LIST NUMBER
One of the options below is applicable depending on whether the item is of military nature or dual-use: *
- For application concerning military items, the code emanates from the EU Common Military List. It should start with ‘ML’, for example: ML9 plus any further subcategories if applicable.
- For applications concerning goods of dual-use nature, the code should be alphanumeric according to the Annex 1 of (EC) Regulation No 428/2009. For example: 6A004 plus any further subcategories if applicable.
BOX 13 VALUE IN EURO
The value in Euro of the total goods under application. Where necessary the conversion rate established by the Customs Department may be utilised. *
BOX 14 QUANTITY OF ITEMS
The total number of items (in units) covered by the application. *
BOX 15 END USE DESCRIPTION
Elaborate on the purpose for which the items will be used. *
BOX 16 CONTRACT DATE
Date of contract (if available).
BOX 17 CUSTOMS EXPORT PROCEDURE
Chose one from the following options: Export, transit, transhipment, intra-community transfer, re-export. *
BOX 18 ADDITIONAL INFORMATION DEEMED RELEVANT BY THE EXPORTER
This section allows the possibility to provide any information to support the application or to put the transaction outlined in the application in context. Any information deemed relevant may be included.
BOX 19 DECLARATION
The declaration may be drawn either by the exporter or the agent. It is to include the date* and the representative’s:
- Signature *
- Full name in block letters *
- ID card number / passport number *
